New Client

If you are a new client to ABT Counseling (never been seen before) or if it has been over a year since your last appointment and would like to get an appointment with one of our counselors, you can click the button below to complete our intake form OR call us at 540-772-8043.

Step 1

Pre-Appointment Paperwork

If you have been given instructions to fill out our New Client Pre-Appointment Paperwork, have your Access ID and Code, provided by our staff, ready then click the button below. The paperwork needs to be completed at least 2 business days prior to your appointment.

Instructions & Notes

We are pleased that you have chosen us in taking this first step towards finding a counselor who works for you. To set up your first appointment, feel free to call our Roanoke office, or complete the New Client Intake (above) to begin the process.

Prior to your first appointment, we will contact you and provide all of the information you need before meeting your counselor. Everything is quick, easy, secure, and confidential to complete online.

We bill most health insurance companies. Our billing experts will check your plan prior to your first session.

 

We are in the process of updating our pre-appointment paperwork so please be understanding through this process.

All pre-appointment paperwork must be completed before your first appointment.

For Telehealth Clients

All forms must be submitted at least 2 business days prior to the scheduled appointment

  • The pre-appointment forms are in PDF format and will be emailed to you for completion.
  • Please complete and print OR save and email this set of forms. It is best to use Adobe not other programs to complete the forms.
  • Please email the forms to us at packet@abtcounseling.com or you can bring them with you to your first appointment.
  • Please follow the instructions given to you by our staff.

New Method

Final Testing
  • Currently this is only being done selectively as it is in the final test phase.
  • The pre-appointment paperwork will be completed online via our portal website.
  • You will be emailed or given a client ID and an access code in order to pull up your paperwork.
  • Once you are finished with all the paperwork and it is submitted, our staff will review and contact you if there are any questions.

We accept the following insurance:

  • Optum (UBH-UHC)
  • Optima Behavioral
  • VA Premier and all Medicare
  • Out of Network

Please check with us to confirm a counselor’s insurance network status.

 

Psychotherapy Fees:

The fee for an initial psychotherapy session is $135. Each successive session is $125. Reduction in fees is considered in some cases. Co-pays, deductibles, and fees are due at the time of service. For your convenience, fees can be paid by credit card, check, or cash in our offices.
